チオリダジン

チオリダジン(Thioridazine)は、フェノチアジン系の第一世代抗精神病薬であり、以前は統合失調症などの精神病の治療に広く用いられていた。重篤な不整脈を誘発したため、先発品は2005年に全世界で販売中止となった。しかし、米国では現在もジェネリック医薬品が販売されている[1]。

    効能・効果

    日本での効能・効果は、下記の3項目であった[3]。

    • 統合失調症
    • 神経症における不安・緊張・抑うつおよび興奮
    • 次における不安・焦燥・興奮・多動 - うつ病、精神薄弱、老年精神病

    重篤な不整脈を引き起こすことから、世界中で自主的に製造中止となった[1][4][5][6]。

    主に統合失調症の治療に用いられていた[7]。また、認知症患者に見られるさまざまな精神症状の治療においても一定の成果があったが[8]、チオリダジンを含む抗精神病薬の認知症患者に対する継続的使用は非推奨だった[9]。

    副作用

    用量依存的にQTc間隔を延長する[10]。多くの第一世代抗精神病薬に比べ、錐体外路系の副作用が著しく少ない[11][12]。クロルプロマジンと比べ鎮静作用が弱いが、抗コリン作用が強く低血圧や心毒性の発現率が高いことも特徴である[13]。他の抗精神病薬に比べ起立性低血圧を引き起こす可能性が比較的高いことも知られている。他の第一世代抗精神病薬と同様に、プロラクチンを上昇させる可能性が高いとされている。体重増加を引き起こすリスクは中程度である[14]。他の抗精神病薬と同様に、遅発性ジスキネジア[注 2]や悪性症候群(抗精神病薬の致命的副作用)の症例と関連している[10]。無顆粒球症、白血球減少症、好中球減少症などの血液異常が生じる可能性がある[10]。他の定型抗精神病薬と同様に網膜変性症との関連が指摘されており[15]、長年の使用により網膜色素異常を引き起こす[16]。稀ではあるが、臨床的に顕著な急性胆汁性肝障害と関連している[17]。

    代謝

    鏡像異性体を持つラセミ化合物である。Eapらによれば、どちらの異性体もCYP2D6によってチオリダジン-2-スルホキシド(メソリダジン)[18]またはチオリダジン-5-スルホキシドに代謝される[19]。メソリダジンはさらにスルホリダジンに代謝される[20]。チオリダジンはCYP1A2およびCYP3A4の阻害剤となる[21]。

    歴史

    日本では1962年2月に承認された[22]。2005年6月に米国・欧州などで販売中止された[1][4]。日本でも2005年12月に販売中止となった[23]。

    抗微生物活性

    超多剤耐性結核菌を殺菌し[24][25]、メチシリン耐性黄色ブドウ球菌をβ-ラクタム系抗生物質に感受性にすることが知られている[26][27]。抗生物質活性の作用機序として、細菌の排出ポンプの阻害が考えられている。β-ラクタム系抗生物質耐性は抗生物質を破壊する酵素であるβ-ラクタマーゼの分泌に起因するため、この分泌を阻害すれば抗生物質は効果を発揮できる[25]。また、肉芽腫性アメーバ性脳炎において、従来の抗アメーバ薬との併用による治療に成功している[28]。
