トリプトリド

トリプトリド(英:Triptolide)はPG490とも呼ばれるジテルペンであり、分子中に3つのエポキシ基を有している。中国原産のニシキギ科のつる植物であるタイワンクロヅルTripterygium wilfordii)に含まれる成分であり、漢方薬としては雷公藤(ライコウトウ)の主要成分である。

トリプトリドは抗腫瘍効果の他、免疫抑制作用[2][3]抗炎症効果[4][5]を持つ。

in vitroin vivo多発性嚢胞腎[6]膵癌のマウスモデルに有効性を示したが、その特性[7]や強い毒性[8]の問題から、治療薬としての使用は困難である。

トリプトリドに代わり、その水溶性プロドラッグであるミンネリド(英:Minnelide)(下記参照)の臨床試験が進行中である[7][9]

作用機序

今までに推定された標的タンパク質としては、ポリシスチン2[10]ADAM10[11]DCTPP1[12]TAB1[13]XPB[14][15]などが挙げられる。 XPB(ERCC3)とその対タンパク質GTF2H4には複数のトリプトリド耐性変異が存在する[16]が、ポリシスチン-2、ADAM10、DCTPP1、TAB1ではトリプトリド耐性変異は確認されなかった。XPBの342番のCysはトリプトリドの12,13-エポキシ基で修飾を受ける残基であると同定されている。XPBのC342T変異はHEK293T細胞株をほぼ完全にトリプトリド耐性とし[14]、それ以前に確認された他の変異と比較して100倍程高いものとなっている[16]。つまり、これらの結果はXPBがトリプトリドの抗増殖活性の標的であることを示している。

スーパーエンハンサーネットワークの崩壊も、作用機序の一つとして示唆されている[17]

腫瘍細胞中やT細胞中では、トリプトリドはDEVD開裂[※ 1]カスパーゼ-3を活性化させてアポトーシスを進行させるが、胸腺細胞中では異なる[18]

トリプトリドはまた、腫瘍細胞内でのTNF-α誘発性アポトーシスの感受性を高めるとともに、TNF-αによるNF-κBの活性化を阻害し[19]、さらにMAPK(分裂促進因子活性化タンパク質キナーゼ)経路のMAPK1を阻害する[20][21]。T細胞中ではインターロイキン-2の発現を抑制する[22]

抗多剤耐性

腫瘍細胞の多剤耐性(MDR)に抗して、トリプトリドは癌化学療法薬の効果を高めるとともに、MDR1/P-糖化タンパク質(P-GP)の発現を下方制御したとの報告がある[23]。またトリプトリドはプロテインキナーゼB/NF-κBの活性を低下させて細胞内の活性酸素濃度を高めることで細胞のアポトーシスを誘導し、同時に多剤耐性関連タンパク質(MRP)の発現を抑制してアポトーシス遺伝子/抗アポトーシス遺伝子(BAX/BCL-2)の発現を調節する[23]

水溶性プロドラッグ

ミンネリドはトリプトリドの水溶性合成プロドラッグであり、in vivo でトリプトリドに変換される[7][24]。膵癌マウスモデルを用いた前臨床試験では、“ゲムシタビンより有効性が高いようである。”

難治性膵癌対象の第2相臨床試験が2019年1月に完了したが、結果は公表されていない[25]

Minnelide

グルトリプトリド(トリプトリドのグルコース誘導体)はミンネリドより水溶性が高く毒性が低い。グルトリプトリドはin vitro ではXPBの活性を阻害しないが、in vivo の腫瘍細胞内では作用する。これはおそらく癌細胞内でトリプトリドが持続的かつ段階的に放出されるためと思われる[26]。グルコーストランスポーターの発現が増加した低酸素癌細胞を標的とする、第2世代のグルトリプトリドが最近報告された[27]
